On July 23, 1982, Vic Morrow and child actors Myca Dinh Le (aged 7) and Renee Shin-Yi Chen (aged 6) tragically passed away during the production of the Time Out segment directed by John Landis.

This segment involved various stunts and spectacles, including a scene where Morrow's character found himself in Vietnam during the war.

In this sequence, Bill Connor had to protect two Vietnamese children while being pursued by a U.S. Army helicopter.

However, during the filming of this action-packed scene, a Bell UH-1 Iroquois helicopter crashed at the filming location in Indian Dunes, Valencia, California.

The helicopter lost control due to poor coordination and reckless planning, ultimately crashing into Morrow and the two children.

The rotor blades of the chopper were damaged by explosions from a debris mortar used for special effects.

The aftermath was gruesome.

Morrow and Myca Dinh Le were reportedly decapitated by the main rotor blades, while Renee Shin-Yi Chen was crushed to death.

Investigations revealed that the children's involvement on the set had been illegal.

They were compensated off the books, allowing the production to evade California's regulations against child labor.

These regulations include the presence of a teacher or social worker and restrictions on working near explosives or during nighttime.
